ICM SAFETY SERVICE AGREEMENT BETWEEN: _____________________________________________________________ __________________________________________________________ __________________________________________________________ hereafter referred to as the user and ICM A/S, tlf.: (+45) 70 606 606 Petersmindevej 15 DK-5000 Odense C Kærvej 25 DK-2970 Hørsholm hereafter referred to as ICM As of today, a service agreement has been made regarding one annual service inspection of the following safety equipment: FIXED FALL PROTECTION PERSONAL FALL PROTECTION GAS DETECTORS RESPIRATORY PROTECTION 1. ICM undertakes the maintenance and service inspection of the personal safety equipment as specified above. 2. The purpose of the inspection is to keep the personal safety equipment in a fully functional and legal condition according to the instructions of the manufacturer and Arbejdstilsynet. 3. The user must always provide ICM with full and unhindered access to carry out service inspections of the personal safety equipment. ICM has the right to perform preventive maintenance with the intention of extending the lifetime of the safety equipment. 4. The price of the service inspection follows the “Price List for Service and Maintenance” for the personal safety equipment. Any use of spare parts for repair or maintenance is invoiced according to the applicable daily price. Prices of service and maintenance follow Denmark’s Net Price Index and are regulated once a year (on February 1st) accordingly. We reserve the right to possible errors in the price list. 5. The service does not include: - Components and installations made by a 3rd party supplier (unless agreed upon). - Repair of damage caused by external accidents, transport accidents, neglect or abuse, as well as other causes than normal use. - Training of staff. Costs of the abovementioned will be charged according to ICM’s normally applicable rates. In case others make changes to the personal safety equipment, ICM can immediately demand to be released from any obligations of this service agreement. 6. This service agreement is valid for 1 year and continues for periods of 1 year at a time, unless the agreement is terminated by one of the parties with 30 days’ notice before the end of a year.
